Have you heard anyone refer to their EHR as "Death by 1,000 Clicks?" The standard EHR template requires doctors to navigate many screens and check boxes to document a patient encounter. Using one of these systems, one physician told us that a normal patient encounter generated 11 pages of notes. Looking closer, his documentation read "Patient presents for pain and/or discomfort in the following areas ..."
So which was it? Did the patient present for pain or discomfort? What you answer now will be what you will have to answer the organizations and legal entities that review your documentation.
